The most obvious thing to do is to connect the SD card to PC using a card reader or if you have a laptop, use an adapter depending on the available slots, then use the standard browsing in Windows to navigate to where your files are on the SD card and copy them to the PC. Reinsert the SD card, but be careful not to delete other files on it and then you can continue working with the device and the SD card. Additionally, use the SleepMapper Data Card Uploader to automatically pull the data from the device and upload it to the supported service. There is also a manual that you can read.

I didn't find anything about the application and the support for SIM cards. The only thing you can attach to the device to read the content data is actually an SD card. There is also a manual for this that can be found on the link posted below.
I've also read the specifications for the device and there was nothing mentioned about a SIM card.
SleepMapper allows you to view the data you've uploaded on your mobile phone or tablet. To do that, visit the official website to get information as well as links for the application. The only way to send the data to your doctor is by actually visiting and showing him the data on your phone or tablet. Based on that he can check the data and stats. This is the only way.

In order to use the program with a new one, to overcome the behavior that you encounter, you need to remove the old one first. To do this, go to Start > right click on My Computer > Properties > Device Manager > select the old device > Uninstall. This will remove the old device thus allowing you to use the program with the new one. You should be able to overcome any difficulties.
